The qualified candidate will oversee members with catastrophic illnesses and specialty illnesses and conditions such as diabetes, HIV and transplants to ensure efficient use of health services. The social worker-case manager will also act as an advocate for the member, coordinating creative solutions to his or her needs.
To apply for this job, candidates should be licensed MSWs with four years’ experience in health care, case management, discharge planning or behavioral health. Work experience with community agencies is also preferred.
